Disclaimers: 1. Applies to existing residential DISH Network customers only. 2. A $5.00 monthly lease fee will be applied to each receiver added to the customer’s account under this promotion. 3. Maximum of two tuners per transaction. 4. Maximum of one model 811 receiver (HD receiver) per transaction; maximun two leased model 811 receivers per account. 5. Maximum four leased tuners per account. 6. Maximum one DISH’n It Up transaction per 12-month period. 7. Customer must provide the following: a. Last name and DISH Network account number as it appears on DISH Network billing statement. b. Valid credit card number for purposes of a credit card validation 8. Customer agrees to maintain minimum qualified programming for 12 consecutive months (Plan A & Plan B) or 24 consecutive months (Plan C) to avoid cancellation fee. 9. Receivers leased by the customer from DISH Network are not eligible for $25 credit and must be returned to DISH Network upon deactivation to avoid any charges. 10. A $4.99 additional outlet fee will be assessed on each dual-tuner receiver (model 625) not connected to a land-based phone line.
